If you have any questions about any of the forms listed here, please call the office. Filling out these forms ahead of your arrival at school will greatly reduce your waiting time.

Forms

All of the forms we generate here can be printed blank, or filled out online and printed with all of the information you entered to submit to the office. By filling them out online and printing them you can save a great deal of time at the school office. Forms that we do not generate may only have print functions.

The only format accepted for a phone number is xxx-xxx-xxxx, the only format accepted for a zip code is five digits xxxxx, the only format accepted for a social security number is xxx-xx-xxxx.

Most of the documents here are pdf (portable document files) files. This format is used because the forms will render (look and act) the same in all modern browsers. It also gives you the ability to fill out and print the forms while online. Arriving at school with forms completed will save you valuable time waiting in the office.

Parent/Guardian/Student Handbook The handbook will provide you with all of the policies and procedures for Christ Our Savior Catholic School.

Web Site Notification Form (08-100) provides detailed information on the parents or guardians use of the web site. By allowing us to provide information to you via the web site, you can help keep tuition costs down. Forms and documents that used to be printed and required extensive supplies and labor, can now be viewed or printed online. Help save the environment, use the internet.

Registration Instructions (08-101) provides detailed information on the registration process and lists all of the forms needed to complete registration. You may also visit the registration section on our web site under "other links".

Registration Form (07-102) needs to be completed for all new and transferring students.

Pre-registration Form (07-103) needs to be completed for all new and transferring students who wish to register for attendance in next years' classes.

Re-registration Form (08-104) needs to be completed for all existing students who wish to register for attendance in next years' classes.

Authorization for Release of Student Records Form (08-105) needs to be completed for all new and transferring students who need to have their existing school records forwarded to Christ Our Savior School.

Family Educational Rights and Privacy Act (07-106) is a document that explains all of the rights of parents and students relative to the release of educational records.

Parent or Guardian Internet Permission Form and User Agreement (07-107) to be filled out by the parent or guardian granting the student access to the internet. The student is also expected to sign and adhere to the school's policies regarding internet usage.

Internet Acceptable Use Policy (07-108) is a document explaining our policy on internet usage at school. It lists the policies and procedures. This information is also included in the school handbook.

Photo Release for Parent or Guardian (07-109) is required by the Archdiocese of Chicago to allow the possible use of photographs of you and your student in promotional material.

Photo Release (07-110) is required by the Archdiocese of Chicago to allow the possible use of photographs of the parent or guardian in promotional material.

State Physical and Immunization Instructions (07-M103) provides detailed instructions for parents and guardians about State mandated health examinations and immunization records.

State of Illinois Approved Physical and Immunization Form (07-M104) the only form accepted to comply with the State of Illinois physical and immunization requirements. Please note that the physician is required to sign this form. Parents or guardians are to complete the students' health history section and sign it prior to visiting the physician. Medication Authorization Form (07-M109) to be completed and signed by the students' physician, allowing the student to access to prescription medication while at school or school sponsored events.

Medical Information and Emergency Notification Form (07-M111) provides the school with a list of contacts to be called in the event of an emergency, if you cannot be reached.

Physician Request for Self-Administration of Medication (07-M112) to be completed by the students' physician, authorizing the school to allow the student to self-administer their own medication.

Parent or Guardian Permission and Authorization for Medication (07-M113) to be completed by parents or guardians, needed to allow the student access to medication while at school or school sponsored events.

State of Illinois Eye Exam Form (08-M-EyeExamReport.pdf) As of January 1, 2008, the State of Illinois has required that all students entering Kindergarten or entering school for the first time shall have an eye examination before October 15th of the school year. State of Illinois Eye Exam Waiver Form (08-M-EyeExamWaiver.pdf) There are three exceptions to the Eye Exam. 1.My child is enrolled in the free and reduced lunch program and is ineligible for public insurance (Medicaid/All KIDS). 2.My child is enrolled in Medicaid/All KIDS, but we are unable to find a medical doctor who performs eye examinations or an optometrist in the community who is able to see the child and accepts Medicaid/All KIDS. 3. My child does not have any type of medical or vision/eye care insurance coverage, and there are no low-cost vision/eye clinics in our community that will see my child.
